Our therapeutic residential care supports young people who need stability, routine and adults who stay — each young person's support is shaped around their needs, culture and goals, aligned with the Hope and Healing framework. We champion school engagement, health, family contact and community activity, and from 16 we plan the skills each young person needs for adult life. We operate within Queensland's child protection framework and the Statement of Standards (s.122, Child Protection Act 1999). Want to join the team? When a young person needs somewhere safe tonight, we're ready. We provide emergency and short-term accommodation for young people in Mackay who cannot safely remain where they are — placements can begin at short notice, day or night, in a clean, welcoming home with trained youth workers on shift around the clock, including overnight. We work alongside the Department of Child Safety and partner agencies, with clear communication and a single point of contact throughout. Referrals: admin@formingpathways.com

Matched to a person, not just a place. In partnership with Our Family Respite, a registered NDIS provider, we offer NDIS Short-Term Accommodation (respite) for children, young people and adults in a new four-bedroom, single-level home in Beaconsfield, Mackay. Our Family Respite provides the registered service and manages referrals; Forming Pathways provides the care team, with residential and out-of-home care experience, personalised care planning and complex behavioural support. Each participant is matched to a designated worker who stays consistent across every visit, with shared, 1:1 and 2:1 support available. Under-18 and adult stays are kept separate.
